Telehealth
Medical Consent
Before a clinician reviews your assessment, you are asked to acknowledge this consent. The plain-language summary below mirrors the in-product consent flow.
What telehealth is
Telehealth uses secure technology — assessments, messaging, and video where applicable — to connect you with a US-licensed clinician without an in-person visit. The clinician treating you is independent of Caliber and is responsible for all clinical decisions.
Scope of care
Caliber supports a focused metabolic protocol (compounded GLP-1 therapy) for eligible adults. Telehealth is not appropriate for emergencies. If you have a medical emergency, dial 911 or go to the nearest emergency department.
Compounded medication
Compounded semaglutide is prepared by a 503B outsourcing pharmacy under cGMP standards. Compounded medications are not FDA-approved. Compounded drugs are not reviewed by the FDA for safety, quality, or efficacy.
Risks and benefits
As with any medication, GLP-1 therapy carries risks — most commonly nausea, constipation, fatigue, and transient appetite suppression, especially during dose escalation. Rare but serious risks include pancreatitis, gallbladder disease, and thyroid effects. Your clinician will review your full risk profile and contraindications before prescribing.
Confidentiality
Your health information is protected by HIPAA. See our Notice of Privacy Practices for how protected health information may be used and disclosed.
Termination
You may decline or stop treatment at any time. Your clinician may also end the treatment relationship for clinical reasons or non- adherence to the protocol.
Your acknowledgment
By submitting your assessment, you confirm that you have read this consent, that the information you provided is accurate and complete, and that you understand the nature of telehealth and the limitations of compounded medication.
Emergencies are not handled via Caliber. If you are experiencing chest pain, severe abdominal pain, breathing difficulty, suicidal thoughts, or any other emergency — call 911.
